A4i Care Team Champion Certificate

A4i (App4Independence) is a digital therapeutic (DTx) and biomarker platform for individuals with severe mental illness. The platform features a mobile application used by patients and a clinical portal to support care coordination by providers. The A4i Care Team Participation on Digital Therapeutics Certificate is awarded to individuals who complete activities in support of A4i for individuals with symptoms of schizophrenia to facilitate successful outcomes for A4i pilot participants in collaboration with the BH Innovations Tech Suite Help@Hand staff. This certificate indicates the successful performance of the following: 

  1. Checking the Care Team Dashboard daily to review any notes from A4i pilot participants
  2. Checking any pending posts that need to be approved on the A4i application
  3. Evaluating the appropriateness of posts and providing the reasoning for why posts may be deemed inappropriate and denied approval for being shared on the app while concurrently tagging inappropriate posts with appropriate hashtags as indicated in the Care Team Manual
  4. Reviewing the progress of the A4i pilot participants on relevant A4i indicators, including their progress on goals, sleep, medication adherence and hallucinations in the Care Team Dashboard on a weekly basis
  5. Monthly meetings with A4i pilot participants to discuss their goal progress and progress on other A4i indicators
  6. Meeting with BH Innovations Tech Suite Help@Hand staff on an ongoing basis per clinic needs to support navigating the Care Team Dashboard and other A4i functionalities
  7. Reaching out to BH Innovations Tech Suite Help@Hand for any questions or issues in relation to the A4i Care Team Dashboard or A4i functionality outside of regularly scheduled meetings with Help@Hand Staff
 
This certificate does not expire and is granted by A4i in partnership with RUHS – Behavioral Health, UCI and CalMHSA
